The objective of a Care Manager is to assist clients and their families in navigating the complicated healthcare system, ensuring continuity of care and the utilization of all available resources.

Serenity is a Family Nurse Practitioner-run concierge service. Our care managers are licensed primary care providers. This high level of medical knowledge bridges gaps to ensure continuity and quality of care. A Serenity care manager can manage medical conditions in the home and work in collaboration with a patient’s primary care doctors. With a vast network of resources, Serenity can meet most medical needs in the home setting, including high-level MD specialists, emergent visits, and diagnostics.
